Introduction
In the field of holistic healthcare, the ancient practice of acupuncture has gained popularity for its therapeutic advantages. While acupuncture has traditionally been connected with pain management and stress reduction, it is being integrated into a variety of Western treatments, including dermatology. The aim of this essay is to explore the advantages of adopting acupuncture into dermatological services, emphasising its ability to improve skin health, treat chronic skin disorders, and increase general well-being.

Chinese dermatology’s view on internal causes of skin health
Acupuncture is guided by several core principles. The first principle is the notion of Yin and Yang, which represent the opposing forces of the universe and the body. Yin relates to coolness, relaxation, and nourishment, whereas Yang signifies warmth, activity, and transformation. Acupuncture seeks to restore the balance of Yin and Yang inside the body, thereby boosting overall well-being and skin health.
Another principle is the flow of Qi, or life energy. According to Traditional Chinese Medicine (TCM), when Qi flows easily, the body is healthy. However, abnormalities or blockages in the movement of Qi can cause a variety of health difficulties, including skin problems. Acupuncture helps to remove these obstructions and promotes the free movement of Qi. Zhao, X. (2006).
Modern principles of Chinese dermatology categorise the causes of disease as either internal or external; even externally caused disease is seen as having an internal relationship. (De Hui, Xin – Fen & Wang. 1995). Understanding how the body’s systems are interrelated, identifying imbalances through diagnostic techniques like pattern differentiation, and using therapies like acupuncture, herbal medicine, and dietary therapy are all part of this. Additionally, contemporary studies are confirming the efficacy of these traditional methods. Jian – Hui. (1993).
The seven emotions
In TCM, the concept of the Seven Emotions, or ‘Qi Qing’, refers to joy, anger, sadness, fear, grief, worry, and fright, which are psychological and physiological responses to one’s environment. Every emotion is associated with a certain organ. Emotional reactions cannot cause disease, but when one of the emotions is triggered beyond the body’s ability to manage it, the corresponding organ is harmed. Zhao, X. (2006).

Chinese dermatology view on external causes of skin health
The Six Excesses
When any of the six climate elements; wind, cold, summer heat, heat, damp, moisture, or dryness, become severe at a time when the body’s protecting qi is low, it might contribute to illness. The symptoms of external excesses are frequently identical to those of internal dysfunction, so diagnosis is crucial for effective treatment.
Wind is supposed to first manifest in the body’s highest and most external portions, including the face, skin, and sweat glands, as well as the lungs. When the body is invaded from the outside, its defensive capabilities are weakened, resulting in a mismatch between the opening and closing of pores throughout the body, allowing the invasion of other pathogenic factors that cause diseases with symptoms such as headaches, nasal obstruction, painful and itching throat, facial oedema, abnormal aversion to wind, and perspiration. Cold is a yin excess that harms yang; cold-related skin conditions are distinguished by purple, darkened skin, a lack of redness and warmth in the affected area, slow resolution of swelling, slow production of pus, and confined localised pain. Choi, L. (2025).
Summer heat is an upward-moving and spreading nature; thus, when it attacks, it causes the pores to open and increases sweating. Increased sweating injures the fluids and allows qi to be lost, causing qi deficiency. Summer heat skin disorders combine with rain to cause dampness, this kind of weather increases sweating even more and can lead to heat rashes and inflammation of the sweat glands. Dampness is more dominant during summer, although it can occur throughout the year with individuals who are exposed to damp, wet or rain environments. Skin disorders associated with dampness include open pores with excess sebum or necrosis of the affected area. Internal dampness is often caused by dysfunction of the spleen’s transformative and transportive mechanisms. Where a build-up of fluid accumulates in the skin, symptoms of intense itching, blistering, swelling and necrosis may occur. Choi, L. (2025).
Dryness often occurs during the autumn months, known as autumn dryness, with the disorder starting in the lungs, and entering through the mouth and nose. There are two kinds of dryness: warm dryness if there are long spells of clear skies without rain and wind and heat dominating the environment. In the west where the wind is cool, individuals will develop cool dryness. Generally, skin diseases are due to warm dryness, which consumes the body fluids, causing chapped lips and cracked skin where harmful bacteria take advantage of cracked skin and enter the body, giving rise to conditions such as carbuncles and furuncles of the hands and feet. Heat often occurs in combination with wind heat, summer heat or damp heat. Skin diseases from heat are sudden or rapid onset, with redness or a burning sensation in the affected area. Symptoms are usually swelling, intense pain, purulence and necrosis, including red or purple macules. In appearance, petechiae can be seen due to heat carrying blood through channels, causing intradermal bleeding. De Hui, Xin – Fen & Wang. (1995)
Differentiation of skin diseases according to the organs
Although the organs are located internally, they manifest externally. Differentiation by organ is based on observing the symptoms of the organ’s physiology and pathology in order to comprehend a disease’s type, aetiology, and location, as well as the balance between normal qi, deficiency and excess components.
Because disease is dynamic and complicated, its characteristics can change to their opposites over time, such as from excess to deficiency, from cold to heat, or from external to internal. De Hui, Xin – Fen & Wang. (1995). Dermatological diseases that are yang, exterior, hot and excessive are usually acute, intensely itchy, rapidly changing and produce local redness, swelling and pain. The patient’s symptoms may present with dry mouth, thirst, constipation, yellow urine, and restlessness with a flushed appearance with a pulse that may be floating, slippery, rapid or strong; the tongue is usually red or has a red tip and greasy yellow coat. Whereas diseases that are yin, interior, cold and deficient are generally chronic, moist and present with locally darkened skin. The patient’s symptoms may present with a bland taste in the mouth, loss of appetite, normal or loose bowel movement or bloating; the pulse may be thin or slow; and the tongue may be slightly pale, swollen or tooth-marked with a white, slippery or sticky coating. Zang Fu organs linking to diseases
Symptoms of Heart and Small Intestine dysfunction presenting on the skin may include red macular eruptions, papules, burning and redness, crusting, pus formation and necrosis. All conditions caused by heat toxin are associated with the heart. If the heat is intense, there will be pain; if the heat is slight, there will be itching. Other symptoms may include ulcers of the tongue, restlessness, palpitations, dry mouth, yellow tongue coating and rapid pulse. De Hui, Xin – Fen & Wang. (1995).
Common skin disorders associated with Heart and small intestine dysfunction are xanthelasmas, boils, furuncles, carbuncles and pemphigus.
Liver and Gallbladder symptoms on the skin may include papules, burning red macular eruptions, necrosis and vesicles. Whenever an emotional upset precipitates the skin disorder, or if the condition appears in the hypochondria, genitalia or near the eyes, then the Liver and Gallbladder are most likely involved; other symptoms include redness of the eyes, anger, depression, a bitter taste in the mouth, a dry throat and a wiry pulse. Common skin disorders associated with liver and gallbladder dysfunction are shingles, tinea cruris and vulvitis.
Symptoms on the skin relating to Spleen and Stomach are seen as vesicles, ulcers, papules and lichenification. Disorders related to dampness arise from a weak spleen yang, linked to a malfunction of the spleen’s transportation function, leading to an increase of dampness. Additional symptoms may include ulcers of the mouth and tongue, poor appetite, loose stools, and diarrhoea. The tongue may present with a greasy coating and moderate pulse. Marenholz, I. Common skid disorders associated with spleen and stomach dysfunction include eczema and various forms of dermatitis.
Lung and Large intestine symptoms of dysfunction on the skin include red macules, papules, pustules and excoriation. As the Lung meridian governs the skin and is also an organ most affected by wind, these skin conditions are usually accompanied by other symptoms such as dry nose and throat and dry cough. The tongue may present with a dry white coat, and the pulse may have a thin, floating, rapid pulse associated with the lung. Yang, J., Guo, J., Tang., et al. (2024).
Common skin disorders associated with Lung and Large Intestine dysfunction include acne, acne rosacea and hives.
Skin symptoms relating to Kidney and Bladder dysfunction, may present as dark patches on the face, pale complexion, and loss of head hair. Other symptoms include forgetfulness, low back pain, tinnitus, excess sweating, chills and fever, bloating, loose stools and or cold extremities. The pulse may present as thin and rapid, with a deep red tongue. Maciocia, G. (2019).
Common symptoms linked to Kidney and Bladder include systemic lupus erythematosus and melasma.
The Four Levels
In Traditional Chinese Medicine (TCM), the skin is thought to be inextricably linked to the body’s internal systems and is evaluated through the lens of pattern differentiation, which analyses symptoms obtained through the four diagnostic methods of inspection, auscultation/smell, inquiry, and pulse. TCM practitioners analyse the skin’s state based on the four levels, Wei, Qi, Ying, and Xue, which describe the depth and severity of a condition, and relate them to the body’s general health and balance. The Wei is the most superficial and is related to the body’s outside as well as the skin’s defence functions. Skin disorders at this level are frequently caused by environmental elements such as wind, heat, or wetness influencing the skin’s surface. Symptoms may include superficial rashes, itching, and minor redness. The treatment focuses on eliminating foreign pathogens and boosting the body’s Wei-qi defensive energy to protect the skin from further invasion. Maciocia, G. (2019).
The Qi level relates to the body’s second layer as well as the movement of vital energy Qi throughout the body. Conditions at this level may include Qi and blood circulation abnormalities, which influence skin nourishment and function. More severe rashes, oedema, and changes in skin texture or colour are also possible symptoms. The treatment seeks to balance Qi and Blood flow, remove stagnation, and feed the skin from within. The Ying nutritive level refers to the deeper layers and the nutrient-dense blood supply.
At this level, there may be deeper inflammation, ulceration, or poor tissue repair.

While the Xus level symbolises the skin’s deepest layer as well as the blood itself and is associated with severe imbalances and chronic illnesses. Conditions at this level may include severe skin disorders, autoimmune difficulties, or underlying chronic diseases. Symptoms may include severe skin damage, systemic symptoms, and considerable impairment of the skin’s barrier function. Treatment entails addressing the fundamental cause of the imbalance, supporting the body’s essential energies, Qi and Blood, and encouraging long-term healing and restoration of skin health. How Does Acupuncture Promote Skin Cell Healing?
The physiologic effects of needling have been extensively studied from the perspectives of cell, tissue, and systems biology; yet, the mechanism of action is only partially understood. The majority of evidence suggests that acupuncture’s effects are mediated by both sensory/neural pathways and the local connective tissues where needles are inserted. Langevin, M L. (2014). The release of growth factors, cytokines, and neurotransmitters mediates certain acupuncture effects. Manni, L. et al. (2010).
Wound healing requires a well-controlled and coordinated balance of immunological defence, epithelial cell proliferation, and differentiation. Wound healing is a multifaceted process that includes inflammation, proliferation, epithelialisation, angiogenesis, and collagen matrix synthesis. Setterfield, L. (2017). In the inflammatory phase of skin restoration, neutrophils and macrophages remove injured tissue and create cytokines, including MIP-1α, IL family, and TNF-α, to speed up the healing process. Singer & Clark. (1999).
Epithelialisation occurs when keratinocytes migrate and proliferate from the wound borders, and stem cells differentiate from the surviving hair follicle bulbs. Chodorowska & Rogus – Sklodowska. (2004). Growth factors secreted by macrophages, platelets, and fibroblasts contribute to collagen deposition. These growth factors include transforming growth factor-β, fibroblast growth factor (FGF), and vascular endothelial growth factor (VEGF). Furthermore, angiogenesis is a multistep process in which endothelial cells sprout from the parent channel and migrate, proliferate, align, and create tubes in additional vessels. Conway, Collen & Carmeliet. (2001). Angiogenesis plays a vital function in wound healing, linked to the expression of angiogenic factors like VEGF and platelet-derived growth factor PDGF. Heo et al. (2011).
It has been demonstrated that relationships between the brain, spinal cord, and peripheral nervous system control pain modulation. Acupuncture also triggers the release of endogenous opioid-like chemicals in the brain. Both serotonin and norepinephrine antagonists block the non-naloxone-responsive component. Han, J S. (2011). Where electroacupuncture (EA) has been applied, the frequency of EA stimulation affects the release of different neuropeptides. While signals from high-frequency acupuncture activate a short parabrachial nucleus (PAG) medulla spinal dorsal horn pathway, signals from low-frequency acupuncture suppress nociceptive transmission by processing in the arcuate nucleus of the hypothalamus β-endorphinergic neurones, periaqueductal grey matter PAG, medulla enkephalinergic neurones, and dorsal horn. Han, J S. (2011). It has also been discovered that adenosine, a neuropeptide with antinociceptive qualities, mediates the effects of acupuncture. Goldman, N. et al. (2012). Acupuncture also affects the limbic system, a key processing hub for the perception of chronic pain.
Treatment for differentiations
Although skin diseases manifest on the exterior surfaces of the body, their occurrence usually reflects the health of deeper physiologic functions, the organs, channels, collaterals, Qi, blood and fluids. In TCM the body is treated as a whole system; skin diseases are treated both internally and externally. Internally using oral herbal medicines and externally to treat local disorders through topical application of the medication. Schmitz, S. (2022). Many skin diseases progress through three stages: prodromal, pustular and ulcerated. Each stage requires its own general treatment strategies:
Elimination of the prodromal stage – clearing heat, resolving dampness and promoting the qi. If pus has already formed, elimination should not be used.
Expulsion for pustular stage – Tonify the qi and blood, supporting normal qi. If no signs of deficiency are present, then medications to discharge pus may be used to prevent the pus from sinking to the deeper level.
Tonification – Harmonising the spleen and stomach and tonifying and nourishing the liver and kidney, qi and blood where lesions have ulcerated, where the pus is clear and less viscous, allows normal qi to new tissue growth to heal the ulcerated lesion. If the toxin is not relieved, tonification should not be done to prevent the spreading of the toxin.
Internal Treatment
Traditional Chinese Medicine (TCM) offers a rich variety of herbal remedies for skin disorders, often based on the root cause of imbalance such as heat, dampness, wind, blood stagnation, or yin/yang deficiency.

Moxibustion for skin disorders
Moxibustion is an important and perhaps underutilised therapeutic method in TCM; it may be used alone or in combination with other modalities such as acupuncture. Li Chan (1575) observed that when diseases treated with herbs or acupuncture are ineffective, then moxibustion must be used. Burning moxa on or above the skin location of specific acupuncture points serves to warm qi and blood in the channels, expel cold and dampness, restore yang and, in general, help to regulate the organs and restore health. Kivity, O. (2020).
Moxibustion therapy consists of two types: Direct and Indirect
At the selected acupuncture point, the moxa cone or rice grain is gently applied to the skin. After that, the cone is lit and left to burn until the skin begins to turn red. The cone is removed as soon as the patient starts to feel warmer. There are two types of Direct moxibustion methods known as ‘scarring’ and ‘non-scarring’. Scarring is where a blister is allowed to form. This method is applied to chronic and recalcitrant disorders such as warts, eczema and psoriasis. De – Hui, S., Xiu – Fen, W., & Wang, N. (1995). Non-scarring where the moxa is removed before a blister forms. Under the moxa, a protective layer is frequently applied. This can be a small slice of garlic perforated with tiny holes placed on the acupoint with a medium-sized moxa cone; the heat penetrates into the acupoint until the skin becomes flushed. Garlic is a local irritant; the area needs to be dried after the removal of the garlic to prevent blistering from excess garlic fluid. Garlic is acrid and warm and is used to kill parasites, relieve and extract toxins and reduce swelling and masses. This method is used for treating carbuncles, furuncles and other skin infections such as mosquito bites. Fen, W., & Wang, N. (1995).
Ginger, whose properties are acrid and hot, helps to regulate the nutritive and protective levels, dispel cold, clear the channels and invigorate the collaterals. Ginger also benefits wind conditions, cold and dampness. Other skin conditions include chilblains and phlegmon due to skin obstruction. Kivity, O. (2020). Creams such as Vaseline or Ching Wan Hung are sometimes used to hold the cone moxa in place or to provide a barrier, while Moxa balm, with its natural ingredients, can be used as a base cream, even with a drop of water or a sprinkle of salt to prevent burns. Pichardo, G. (2023).
Pre-rolled moxibustion sticks used as indirect moxa treatment are thought to be safer; they are used more frequently. By holding the burning moxa stick above the body until the skin becomes red and heated, never letting it come into direct contact with the skin. Pichardo, G. (2023).
Case study 1: Eczema
A German pilot study found that acupuncture reduces itch severity in patients with atopic eczema, which coincides with reduced basophil activation. Ten patients with atopic eczema were randomly assigned to acupuncture or no therapy. On days 15 and 33 (after 5 and 10 acupuncture treatments, respectively), the acupuncture group had considerably lower mean itch intensity on a visual analogue scale. At all-time points, basophils from the acupuncture group demonstrated considerably lower in vitro activation upon allergen stimulation than the control group. (Altern, J. 2011).
Case study 2: Acne
Forty cases of acne were treated by acupuncture plus herbal drugs (acupuncture and drug group, A) and 30 cases were treated by simple acupuncture as the control group (acupuncture group, B). The results showed a basic cure in 31 cases, a remarkable effect in 5 cases, an effect in 3 cases, a failure in 1 case and a total effective rate of 97.5% in Group A, and a basic cure in 16 cases, a remarkable effect in 6 cases, an effect in 3 cases, a failure in 5 cases and a total effective rate of 83.3% in Group B. There was no significant difference (P>0.05) in the therapeutic effect, and there was a significant difference (P<0.01) in the curative rate in the two groups. The therapeutic effect was obviously better in Group A than in Group B. (An, He. C., Xi ping, D., & Hai bo, H. 2003).
Acupuncture has also shown promise in reducing itch symptoms, with multiple studies demonstrating that it is useful in treating both acute and chronic itch, notably in atopic dermatitis. Acupuncture, according to research, can lessen itch severity and skin responses while also improving patient quality of life. A systematic review showed the meta-analysis of therapeutic effects that apply acupuncture therapy to treat itch-related disease by analysing all the collected data from three RCTs involving 70 individuals. The result revealed that acupuncture is effective to ameliorate the itch intensity of itch-related disease. The summary mean difference (MD) was 19.03 (95% CI: [8.09, 29.97], Z = 3.41, and P(Z) = 7 × 10−4) by using a random-effects model. A significant estimate was observed to support that acupuncture could be effective to inhibit itch intensity. Yu, C., Zhang, P., Lv, Z T., et al (2015).
Conclusion
Acupuncture is a comprehensive approach to healing that goes beyond simply treating symptoms. It targets the underlying root causes of imbalances and intends to restore the body’s natural balance. Acupuncture can benefit the skin by increasing overall health and well-being, allowing for a healthier complexion and quality of life. Although further research is needed, acupuncture’s application in cutaneous medicine appears to be promising in terms of itch reduction, atopic dermatitis and herpes zoster pain treatment, and wound healing promotion.
The skin is frequently regarded as the major defence mechanism and barrier against many infections of the body. Maintaining good skin is essential for general well-being, and it can be accomplished with a combination of current allopathic and natural therapies. Common skin disorders such as psoriasis, eczema, and acne provide substantial healthcare issues. Plant and animal extracts have been shown to effectively cure skin infections.
With the advent of science and technology, the clinical need for traditional Chinese medicine has grown dramatically.

As a result, natural herbal remedies for skin disorders have emerged as essential components in the treatment of skin infections. Traditional Chinese medicine extracts have made significant contributions to human health.
In recent years, there have been substantial advances in the study into the functional parts of traditional Chinese medicine, with an increasing number of components demonstrating potential for preventing and treating skin disorders. These elements, which are fundamental to traditional medicine, are gaining acceptance in modern medical practice. Future research should focus on identifying the mechanisms of action of these functional components and testing their efficacy in clinical trials. This will encourage the use and advancement of traditional Chinese medicine in treating skin diseases.
